Identify the decision-maker

The person who decides whether to write a prescription is the prescriber. Ask for that person’s full name, job title and state licensing board.

Federal rules recorded September 26, 2026 require a prescription from a licensed person allowed to prescribe before a prescription medicine is supplied.

Ask whether your assessment could lead to a different option or no prescription.

Look for a conversation that can respond

The Federation of State Medical Boards advises boards on online assessment.

Its policy rules out treatment decisions made only from a form that misses health information needed for the assessment.

Its policy requires the doctor to be able to ask more questions or obtain more of your health history.

It permits online assessment when it meets the care requirements for an appointment in a room; rules differ from state to state.

These recommendations for state boards were checked September 26, 2026; they are not a nationwide law about questionnaires.

Ask how missing details are handled

  • Can the person assessing me ask more questions after reading the form?
  • How can I raise a question before a prescription decision?
  • What would mean an examination or test is needed?
  • Who explains the decision if no medicine is prescribed?

Assessment, in plain terms

Look for a decision about your concern, with room for questions in both directions.
