The payer and the paperwork

Ask the plan about the actual offer

Give the insurer the full product name, the proposed form and the reason given for using it. Ask what information the plan needs before it can answer.

  • Is this product covered for this proposed use?
  • Is advance approval needed before payment?
  • What part would I pay?
  • Are visits, tests and the product handled separately?

Keep the answer in writing. Ask the care service which charges remain if the plan declines payment.

Ask separately about medical spending accounts

An FSA, or flexible spending arrangement, pays eligible medical costs; so does an HSA, a health savings account.

Under the tax rules recorded September 26, 2026, being beneficial to general health alone does not make an expense qualifying medical care.

For weight-loss costs, the Internal Revenue Service (IRS) gives a specific example: the expense must treat a particular disease the doctor has diagnosed.

IRS is the federal tax agency. That example shows why to ask about the purpose of the care. For an answer about your product, ask the person running your account about the actual expense and records needed.

Ask who needs which document

A “letter of medical necessity” explains why the care is needed; the person running your account may request that letter.

The IRS documents checked September 26, 2026 do not name that letter as a requirement.

Ask who must write the letter and what it must say before counting on getting money back. Open “Read the source material” for the IRS links.

The answer to retain

Keep the payer’s product-specific answer and document request with the written quote.
